Annual Progress Report Form
Student fills out the form before meeting with their Guidance/Dissertation Committee for their Annual Research Progress Evaluation Meeting.

Annual Research Progress Evaluation Form
Once the student meets with their Guidance or Dissertation Committee, the student will submit this form. The form will then go to the committee chair for it to be filled. Once the committee submits the form, it will go to the program's Graduate Advisor for their signature. Once these steps are complete, the form will go to Graduate Division.
 

GGB Dissertation Defense Announcement Request Form
Student should submit request at least 2 weeks before Dissertation Defense. Student Services Advisor will email the announcement out to the GGB Faculty and Students.

Form AA - Courses Program Form
The program's Graduate Student Services Advisor will fill out the form for the student. The form will then be placed in the student's file for the program's Graduate Advisor to view.

Form BB - Courses Plan Form
Students can use this form to plan out their courses each quarter when meeting with their Major Professor or Rotating Professor

Form CC - Other Courses Taken
The program's Graduate Student Services Advisor will fill out the form for the student. The form will then be placed in the student's file for the program's Graduate Advisor to view.

Form DD - Guidance Committee Approval Form
By the end of the first quarter of residence, the student should submit this form to nominate their Guidance Committee.

Form EE - Student Progress Record for Graduate Advisor and Student
Students should use this form to keep track of their progress toward their degree.

290 Petition - Directed Studies

Directed Study (not research; research = units of 297 or 299).  

Deadline: first day of instruction.*

Add a Major

Current graduate students can request to add a major with the approval of the new program and their current program. Please visit Graduate Admissions.

Change in Degree Objective - PhD to Master's

 

Current graduate students may change from PhD to Master's by using this petition.  This form is due by Friday of week 3 of the quarter.*

Change in Degree Objective: Master's to PhD Students who want to change from Master's to PhD must follow the Graduate Admissions instructions.

Change in Major

Current graduate students can request to change to another major with approval from the new program.  Students must apply to the new program by the deadline using the instructions from Graduate Admissions.

Credit by Exam

Students who wish to receive credit for a course by formal examination without enrollment in a course.  This form is due by the first day of instruction.* $5 fee applies.

BS/MS students only - identifying courses from undergraduate work to count at the graduate level. 

Education Abroad Program Academic Planning Form for Graduate Students 

Advising form for graduate students who wish to pursue Education Abroad Programs.  Students must start by consulting with the UCR Study Abroad office

Enrollment Adjustment Form

Students wanting to make changes to enrollment after week 2 of the quarter.* After week 3*, a $4 fee applies.

Grade Appeal

Appeal a grade by submitting form to the Academic Senate 

Half Time Status and Reduced Fees

Students who cannot attend more than half time due to a full time job or for medical or personal reasons.

In Absentia Registration

Research or study requires extended stay outside California (for the duration of a quarter).

Incomplete Extension Extending an incomplete grade beyond one quarter.

Increase Unit Limit

Increase the maximum units for a quarter above the 20 unit limit.

Intercampus Exchange Application

Take classes at another University of California campus.  This petition is due at least four weeks prior to the beginning of the quarter. 

Fall: September 1
Winter: November 15 (considering holiday campus closures)
Spring: March 1

Letter of Completion

Request a letter stating that you have completed all degree requirements. You may request this letter if you have an immediate need to show proof of degree completion prior to the transcript and diploma being available and if your degree audit in R'Web is showing 100% complete.

Leave of Absence

Request 1-3 quarters of leave from studies.

Manuscript Completion (Creative Writing Only)

Creative Writing and Writing for the Performing Arts students reporting completion of the manuscript.

Deadline: last day of the quarter.*

NRST Reduction Extension Request

Request to extend the 9-quarter reduction after advancement to candidacy to 10 quarters.

Readmission

Students who were enrolled in a graduate program for one quarter or more and have withdrawn from the University can apply to re-admit back into that same graduate program.  Students must apply by the deadline using the instructions from Graduate Admissions.

Remote Participation (NOT CURRENTLY IN USE) Request that one member of the oral qualifying exam committee or the thesis/dissertation committee be permitted to participate in the exam remotely

S/NC Petition (USE ENROLLMENT ADJUSTMENT)

Take a course for S/NC credit: this petition is only necessary for courses numbered 100 and above.  001-099 (lower-division courses) may be taken for S/NC credit without a petition (if S/NC is a cataloged grading option).

Substitute Coursework

Students wanting to substitute one required course for another or one non-course requirement for another.

Transfer Coursework

Students wanting to transfer coursework from another university or program to their UCR transcript.

Waive Course Work

Waive a degree requirement based on previous academic work.

Withdrawal from UCR

Withdraw from the University.

PhD Advancement and Graduation Forms

Nomination for Oral Qualifying Exam Committee

Nominate the oral qualifying exam committee at least 2 weeks prior to the exam.

DEADLINE: At least two weeks (preferably one month) prior to the Oral Qualifying Exam date.

Report of Oral Qualifying Exam (and Nomination of Dissertation Committee)

Report the results of the oral qualifying exam & nominate a dissertation committee.

DEADLINE: This form should be initiated within 48 hours of the completion of the exam.

Committee Change (Dissertation or Thesis)

Nominate or make a change to the dissertation or thesis committee. 
 

DEADLINE: At least two weeks prior to the Final Defense date or as soon as the change is confirmed by the department.

Final Defense PhD

Report the results of the final defense of the dissertation.
